START FLPB

The following paperwork is needed to start FLPB:

 DA Form 330 Language Proficiency Questionnaire generated at the DLPT Testing Center.

 DA Form 4187 Personnel Action with appropriate “A” List FLPB amount and BN Command Level Signature.

 Orders with BN adjutant signature granting FLPB generated at the BN S1.
